We are now Optimal Home Health & Hospice.
While our name and digital presence are coming together under one brand, the things that matter most have not changed. You’ll still work with the same trusted team. You’ll still receive the same personalized, compassionate care. And you’ll still have open, honest conversations about how we can help you in your journey.
This change simply makes it easier to understand how we can support you, whether you’re focused on recovery, maintaining independence, or navigating more advanced care.

Our Story
Optimal began with a simple idea: help people in a way that truly makes a difference.
In 2005, two close friends, Edwyn and Chad, moved to Colorado to build something rooted in that belief. With backgrounds in healthcare, business, and a shared commitment to their community, they started a home health agency with just a small team and a lot of determination. It wasn’t easy in the beginning, but through persistence, faith, and support from those around them, the organization steadily grew - always grounded in the idea that people come first.
Years later, another part of the story began to take shape.
The hospice side of Optimal started separately in 2010. In 2013, Eloise became involved in helping manage the organization. She believed strongly that home health and hospice should work together—that patients would be better supported if their care felt connected, especially during times of transition.
In 2018, everything changed.
Eloise was diagnosed with stage four pancreatic cancer and passed away just a month later. During that time, her experience with hospice brought a new understanding of what this type of support truly means—not just clinically, but personally. Before she passed, she asked that the hospice side continue.
That request became something much deeper than a business decision.
Through her journey, and later through the care of a loved one, it became clear how important it is for patients and families to feel supported, informed, and not alone during some of life’s most difficult moments.
Over time, the home care and hospice sides were brought together—not just operationally, but intentionally. What started as two separate paths became one connected approach.
Today, Optimal reflects that full journey.
It’s built on the belief that support should feel personal, that communication should be clear, and that no one should have to navigate changes in health alone. From recovery to ongoing support to end-of-life, the focus remains the same: to walk alongside patients and families with consistency, understanding, and care that reflects who they are.

We provide ongoing grief support for families for up to 13 months, offering guidance, check-ins, and resources to help you navigate loss in your own way and at your own pace.

Each session is led by a board-certified music therapist and is tailored to the individual. Using live music, sessions may support relaxation, improve mood, encourage connection, and provide space for reflection and meaningful moments.
