Understanding Hospice Eligibility for ESRD

Hospice eligibility for patients with End-Stage Renal Disease is based on clinical findings, disease progression, and the patient’s goals of care. Clinical judgment always remains an important part of determining eligibility.

In general, one of the following conditions is typically present:

1. The Patient Is Not Seeking Dialysis or Renal Transplant

Hospice care may be appropriate for patients who choose not to pursue:

  • Dialysis treatment

  • Kidney transplant

  • Aggressive curative interventions

OR

2. The Patient Is Discontinuing Dialysis

Patients who decide to stop dialysis treatments may qualify for hospice services focused on comfort and quality of life.

Clinical Indicators Supporting ESRD Eligibility

Medical records may support hospice eligibility when advanced renal disease is accompanied by one or more of the following findings:

Kidney Function Indicators

  • Glomerular Filtration Rate (GFR) less than 15%

  • Creatinine clearance less than 10 cc/min

    • Less than 15 cc/min for diabetic patients

  • Serum creatinine greater than 8.0 mg/dL

    • Greater than 6.0 mg/dL for diabetic patients

These findings indicate severe kidney dysfunction and declining renal function.

Secondary Conditions That May Support Eligibility

Additional complications related to advanced kidney disease may further support hospice eligibility.

Common Secondary Conditions May Include:

  • Hyperparathyroidism

  • Calciphylaxis

    • Calcium deposits in blood vessels that may cause painful skin ulcers or tissue damage

  • Nephrogenic Systemic Fibrosis

    • Hardened or thickened skin changes

  • Electrolyte abnormalities

  • Other renal-related complications

These conditions can significantly impact comfort, mobility, and quality of life.

Comorbid Conditions

Many patients with ESRD also experience other serious health conditions that contribute to overall decline.

Common Coexisting Conditions May Include:

  • Coronary Heart Disease (CHD)

  • Peripheral Vascular Disease (PVD)

  • Congestive Heart Failure (CHF)

  • Anemia

  • Vascular Dementia

  • Other chronic illnesses

The combination of ESRD and additional medical conditions may increase symptom burden and care needs.

Signs and Symptoms of Advanced Renal Disease

Patients with advanced ESRD may experience symptoms that affect both comfort and daily functioning.

Common Symptoms May Include:

  • Unintentional weight loss

  • Fatigue and weakness

  • Headaches

  • Frequent hiccups

  • Itching (pruritis)

  • Peripheral neuropathy

  • Changes in skin pigmentation

  • Decreased or absent urinary output

  • Easy bruising or bleeding

  • Muscle twitching or cramps

  • Nausea or vomiting

  • Seizures

  • Bone weakness, fractures, or joint disorders

  • Gastrointestinal bleeding or ulcers

An important part of hospice eligibility includes understanding the patient’s personal wishes and goals for treatment.

Some patients and families choose to focus on:

  • Comfort and symptom management

  • Spending meaningful time with loved ones

  • Reducing hospitalizations and burdensome treatments

  • Emotional and spiritual support

  • Quality of life and dignity
