Each member of the hospice team plays an important role in addressing the physical, emotional, social, and spiritual needs of patients throughout the hospice journey.

Registered Nurse (RN) Case Manager

Your hospice nurse serves as the RN case manager and helps coordinate all aspects of care.

Responsibilities of the RN Case Manager

The nurse will:

  • Assess physical, emotional, and symptom management needs

  • Monitor changes in the patient’s condition

  • Coordinate care with physicians and healthcare providers

  • Provide education regarding:

    • Medications

    • Symptom management

    • Disease progression

    • Comfort-focused care

  • Help ensure the patient’s goals and wishes are honored

  • Provide emotional support and guidance for families and caregivers

The RN case manager also helps ensure communication among all members of the healthcare team so care remains personalized and consistent.

Medical Social Worker

Hospice social workers help patients and families navigate the emotional, practical, and social challenges that often accompany serious illness.

Social Work Services May Include:

  • Emotional support and counseling

  • Assistance coping with serious illness and life changes

  • Help identifying community resources

  • Guidance with advance directives and healthcare planning

  • Assistance completing social service or healthcare forms

  • Support with future planning and caregiving concerns

Social workers help families feel supported, informed, and connected to available resources.

Spiritual Care and Chaplain Services

Hospice care supports emotional and spiritual well-being in addition to physical comfort.

Spiritual Support May Include:

  • Prayer or spiritual guidance

  • Emotional support and compassionate listening

  • Presence during difficult moments

  • Support for questions related to meaning, faith, grief, or peace

Hospice Aides

Hospice aides are valuable members of the interdisciplinary care team who assist patients with personal care needs while promoting comfort, dignity, and cleanliness.

Services Hospice Aides May Provide

Hospice aides may assist with:

  • Bathing

  • Grooming

  • Hair care and shampooing

  • Oral care

  • Shaving

  • Dressing

  • Skin care

  • Changing bed linens

  • Mobility assistance

  • Light housekeeping related to patient care

Hospice aides work under the supervision of the registered nurse and follow the individualized care plan developed for each patient

What Families Can Expect from Hospice Aide Services

Communication and Support

The aide will:
